Buyer's guide · Philippines
Hospital information systems in the Philippines, a buyer's guide.
What a hospital system has to get right for a Philippine private hospital, the questions to ask any vendor, and what decides the price.
In one paragraph
The short answer.
A hospital information system in the Philippines runs a hospital from the emergency room and admission to the doctor's orders, nursing, laboratory and pharmacy, and then the bill, the discharge and the PhilHealth and HMO claims. It is used by admitting, nurses, doctors, the laboratory, the pharmacy, the cashier, accounting and the administrator. It must deduct PhilHealth benefits by case rate and file claims through eClaims under the Universal Health Care Act (RA 11223), support the hospital's DOH licence, and protect patient records as sensitive personal information under the Data Privacy Act (RA 10173).

Part 1
What it must do from admission to discharge
A hospital charges as it treats. Every medicine dispensed, test run, room day and procedure has to land on the right patient's account the moment it happens, or the bill at discharge is wrong and the claim is short.
- Emergency room visits, admissions and a bed board that shows which beds are free.
- Doctors' orders, the medication administration record, nursing notes and vital signs.
- Laboratory and imaging worklists, and results with normal ranges.
- Pharmacy dispensing by lot and expiry, returns, and the dangerous drugs register.
- Operating room schedules and cases.
- Charges as they happen, the statement of account, discharge clearance and the gate pass.
Part 2
PhilHealth: case rates and eClaims
PhilHealth pays an accredited hospital a fixed amount per case, by the diagnosis or procedure, rather than item by item, under the Universal Health Care Act (RA 11223) and its circulars. The benefit is deducted from the patient's bill at discharge, and the hospital then claims it through eClaims, generally within 60 days of discharge.
For eligible patients in basic or ward accommodation, the no balance billing policy means the patient pays nothing beyond the package. A system should apply the right case rate, show the deduction on the bill, track each claim to payment or return, and warn before a filing deadline passes.
Part 3
Other rules a hospital bill must follow
Several laws shape how a hospital bills and discharges, and the system has to make them easy to follow rather than easy to forget.
- RA 10932 forbids requiring a deposit before giving emergency care.
- RA 9439 forbids detaining a patient for unpaid bills when a promissory note is signed; the law does not cover patients in private rooms.
- RA 9994 and RA 10754 give senior citizens and persons with disabilities a 20% discount and VAT exemption.
- RA 9165 requires dangerous drugs to be accounted for, including wastage.
- Doctors' professional fees are billed apart from hospital charges and paid out to the doctors.
Part 4
DOH licensing
A hospital operates under a licence from the Department of Health under the Hospital Licensure Act (RA 4226), and its laboratory, pharmacy and other services have their own licences. A system should keep each licence and its expiry, and the reports the DOH asks for should come from the records, not from a separate count.
Part 5
Patient privacy and why paper does not scale
Charts, results and diagnoses are sensitive personal information under RA 10173. Only the care team should open a chart, every opening should be logged, and a breach must be reported to the National Privacy Commission. Paper charts and spreadsheets cannot show who saw what.
A hospital also has more people writing to one record than any other business. A system that keeps orders, charges and claims on the same patient account is what keeps the bill and the claim right.
Before you buy
Questions to ask any vendor.
- Does every charge reach the patient's account the moment it is ordered or dispensed?
- How does it apply PhilHealth case rates and no balance billing on the bill?
- Does it prepare eClaims and track each claim to payment, with deadlines?
- Can it take a promissory note at discharge instead of holding the patient?
- Does it keep the dangerous drugs register and pharmacy stock by lot and expiry?
- How are doctors' professional fees declared, billed and paid out?
- Who can open a chart, and is every opening logged?
- Do we own the data and the source code, and can it run on our own servers?
Cost
What decides the price.
Every business is priced on what it needs. These are the things that move it.
Beds, departments and users
A larger hospital has more wards, stations and staff to set up and train.
Services covered
Laboratory, imaging, pharmacy, the operating room and the emergency room each add their own records.
Payers
PhilHealth packages and each HMO's rates and claim process add setup.
Old records
Bringing in patients, open accounts and the price list from an old system is part of the work.
Devices and connections
Laboratory machines, barcode printers and other equipment each need a connection.
Hosting
Servers in the hospital or a cloud server looked after by the vendor.

Questions
Asked and answered.
What is the best hospital information system in the Philippines?
The best one charges as care happens, applies PhilHealth case rates and files eClaims, follows the laws on deposits and detention, keeps charts private and logged, and leaves the hospital's data with the hospital. Compare vendors on those points.
How much does a hospital information system cost?
It depends on the number of beds and users, the services covered, the payers, the old records to bring in, the devices to connect and the hosting. A vendor should price it in writing after a meeting on the hospital's own process.
Is a hospital information system required by PhilHealth?
PhilHealth requires accredited hospitals to file claims electronically through eClaims, which in practice needs a system that can produce the claim data. No single vendor is required.
Can a hospital refuse to discharge a patient who cannot pay?
Generally no. RA 9439 forbids detaining a patient for unpaid bills once a promissory note, secured by a mortgage or a guarantee, is signed for the balance. The law does not cover patients who stayed in private rooms.
